
CONTINUUM OF CARE
The Continuum of Care (CoC) is designed to guide community-wide planning and strategic use of resources to address homelessness. Threshold CoC leads this effort by convening leadership to collaborate on best practices.
By using common data and assessments, we prioritize and effectively direct people to the most appropriate support for their level of need. We gather data, determine priorities, improve integration and performance measurement, and tailor programs to the strengths and challenges in each community.
This includes:
Sharing common processes for intake and assessment
Prioritizing those with highest needs
Providing emergency shelter
Identifying creative housing solutions
Extending wraparound support
OUR WORK IN THE CoC
The Continuum of Care (CoC) Program is designed to promote community-wide commitment to the goal of ending homelessness by providing funding for efforts by nonprofit providers and State and local governments and promoting access to and effect utilization of mainstream programs by homeless individuals and families.
Threshold CoC holds monthly General Membership meetings via Zoom with all CoC partners the 2nd Wednesday of each month from 11am – 12:30pm. Meeting agendas and recordings of past meetings may be found in our General Membership SharePoint folder.
